New Student Enrollment


To enroll a new student at Canyon Middle School you will need to complete the two Phases listed below. 
Many of the forms needed for enrollment are available by clicking the link.

Students attending another CVUSD school may register by completing the residency.  Records will be requested from CVUSD schools and must include copy of birth certificate and up-to-date immunizations.

Students new to the district are to complete Phase I and Phase II.  Phase II is to be completed within 30 days of completing Phase I. Save time by completing Phases I and II in one visit by bringing in all the required paperwork.  Students will not be scheduled based only on residency.

6th Grade - "0" Period Music Elective (pdf)

Parent/Guardian to provide:

Birth Certificate, Immunization record, and other educational needs information, Copy of latest report card, Copy of STAR or any other test scores

Students entering a Canyon Middle School from another state or another country MUST have the required immunizations completed.
